Especially if i'm going into a hospital. I make sure they know that I don't want any religious nuts walking into my room.

I think I put Unitarian on a hospital form decades ago. Then when I was admitted years later a steely-eyed holy roller pastor barged into my room and demanded to pray for me. Perhaps she didn't know that most UUs don't pray or she was hoping to convert me.

I had that information removed from my record and told the hospital to never let any pastoral care person in my room if I'm ever there again.

Last Monday I had to go to the emergency room and was admitted for a two day stay. DH reminded the nursing staff that I didn't want spiritual counseling and no one showed up this time, so I think the situation is under control.

Perhaps the military asks about religion? Insurance companies? I think it would be unlawful to include this question on most forms,

I don't think I've ever filled out a form that asked for "religion."

I would probably leave it blank or put "n/a."

I've only ever heard of the military asking that question or hospitals asking. Fill in your denomination or "Christian" only if you want pastoral care. If you want to be left alone, put N/A.

The only time I've seen this is on hospital forms. Put what you feel comfortable with. Many hospital chaplains are used to agnostics or "others" who just want to talk. If you just want to talk, gently mention that and the person should respect your wishes. Remember that many religious groups have clergy/clerics who are trained to help with grieving or palliative care. Very best wishes, Boner.

Technically, I'm a Presbyterian, but I haven't attended there in ages, and I wouldn't want to bother the kind young minister with having to visit me. I like the guy, but hey- I don't contribute anything to his church and we both know it. He has enough to worry about.

If I'm either ill enough to be in the hospital, or especially, if I'm post-operative and grouchy with pain, I don't want to have to put on party manners for anybody. My family understands this.

My son, bless his heart, was sitting in a chair at my bedside when I awoke after surgery. He said he had sent DH home, and was assuming duty. I was still pretty out of it from anesthesia, but tried to carry on a proper conversation. He finally put a hand on my arm and said, "Look, Mom - you don't have to entertain me. you've been through a pretty rough surgery, you're still pretty gronked-out on drugs, so just go back to sleep. I'll be here when you wake up." And he was.

Can you imagine the love of a son who is willing to sit and read, while you swim up from anesthesia? I was willing to sink back into sleep, waking up later much less foggy-brained, and yes, Son was still there. I am so grateful for my kids, the ones I can laugh with, cry with, and just enjoy BEING with.

Son told me later that the nurses had tried to shoo him out after "visiting hours." He explained, very politely, that he had promised, during my brief period of post-op wakefulness, that he had promised to be there the next time I woke up, and he intended to keep that promise, but he assured them that once I was fully awake, he would go home. But he wasn't about to let him Mom wake up to an empty room after making that promise.

The nurses were very understanding. Several of them told me I had a very sweet son, and I just smiled, saying, "Yeah, he has always been that way." Lucky, lucky me!!

I don't know about the US but in the UK it's asked on the census forms.

When I was recovering in the hospital from my emergency
appendectomy a hospital chaplain dropped by for a short visit.
It was very pleasant and religion was not mentioned by either of
us.

This question is on census forms in Australia. OH and I just put " None".

The percentage who put " None" is getting higher with each census.

Many years ago I was admitted to a hospital and was asked about my religious affiliation. I said I was an apadiest. Apparently they didn't have that check box so I said none.

Believe it or not, when I had my heart attack 2 weeks ago, NO ONE anyplace asked me about religion. Not in the local ER or the farther-away hospital to which I was sent via helluva-copter, and not during my stay in the hospital. OK by me!

The only time I've encountered this was on the preliminary paperwork at a doctor's office. I wrote "Christian" for the same reason you did.

Then again, I knew this lady who was high as a kite when she was admitted to a detox ward. She said she was a Druid.

When I was admitted to the hospital on Christmas Day a couple of years ago (bad pneumonia), on the "religion" line, I put Atheist U-U. My UU minister and his wife came up the next day. I was glad to see them--they were the only people who came that entire week except my best friend. (None of my kids/grandkids have cars...)

If I ever have to go again, I plan to put Satanist and see who shows up. LOL! :)

If it is a hospital admission form (which is the only time I have ever encountered this request), I put "Jewish"...

...on the theory that whoever Jewish that stops by (be it a rabbi, or a rabbinical student, or whoever) is going to be a MUCH better conversationalist than most of the other hospital personnel I will encounter. (I will be able to, more or less, interview THEM, and learn some neat inside stuff I wouldn't have learned without them stopping by. Can't ever tell when that exact kind of info will be PRECISELY what I need for [including in] a book or whatever.)
