Well..., really Last place, but that is still #1 in Utard language.

The way of The Lord is 10 percent. And who can tip much with the club costs?

Anecdotal experience of mine also suggests Utah has the highest restaurant ratio per person. They are flooded with them.

The dreaded moment for every fast food worker.

"May I take your order".

Reply, "Yes, can I have 12 hamburgers with just ketchup, put a pickle on two, be sure to not salt them. Oh and 14 small waters."

As usual, I wonder why this is.
Is it that Mormons are CHEAPSKATES?

Almost anyone who has ever invited both Mormons and non-Mormons to a wedding and/or reception knows the answer to your question.

My daughter's friend was a waitress while she was going to college. Worked in a nice place. She said that you wouldn't believe the low tips. Always from people that never ordered alcohol. She heard "I don't give the lord more than 10%, I'm not tipping more than that". Like it's a badge of honor. Some people don't belong in public.

A major part of the problem is that Mormons are OK about paying for things, but they hate to pay for services. The church trains them to feel that way. A whole bunch of services at the ward level are expected to be done for free.

At other churches, janitors are paid, office staff are paid, organists and choir directors are paid. Ministerial staff are paid. There are still plenty of volunteer hours rendered (coffee hour, ushers, choir members, various committees), but jobs that take multiple hours per week, and happen on a regular basis are paid.

So Mormons look down on service work and expect it to be free.

On top of that tipping traditions in the US are pretty absurd by the standards of the rest of the world (both in how much is an appropriate tip, and who should be tipped), and are getting worse, with payment devices asking how much you are tipping, often at the time you order, and while the server is watching. People are starting to rebel.

" . . . with payment devices asking how much you are tipping, often at the time you order, and while the server is watching."

This is the one that is really too far. You don't even know if the service is going to be good and we have all had some really really bad service. So now the actual service is beside the point?

But I have snuck the waiter some extra money when I knew the person treating us to dinner did not tip well--or at all. Sometimes because they were from a country which did not tip.

The tipping system is broken like everything else in this country. Having been raised BIC pales in comparison with what else I face daily now.

Ditto. The tipping thing with the credit card question on the screen is really irritating.
Then they sometimes ask if I want to donate to something else as well.
Cash is my preference for places like that.
I try to tip in cash separately when possible directly to the person who provided good service.

The entire system is frustrating.

Using cards the server only receives about 97% of what you intend, banks & processing take their cut off the top:

If servers don’t pay (Federal) income tax on tips (what about cashiers, cooks, & other staff?) doesn’t that effect their Social Security & other items?

Yep. Plus some restaurants make all their servers pool their tips and all get an equal cut. The good ones subsidize the low tip earners. We always bring cash. Pay the bill with a points earning credit card and zero the tip. Leave them a cash tip so hopefully it all goes to them.
