Your request seems to be a little naive, as summarizing the history of Mormonism AND its problems into one page is basically impossible, but this page by long time ex-Mormon Richard Packham is probably the closest thing for what you are looking for. He summarizes the basic history and theology (at least what is taught by the missionaries) and then explains the other, problematic side of it that is NOT taught:

www.MormonThink.com is an excellent website explaining many of the problems. It is written by several current and former Mormons.

Jerald & Sandra Tanner, some more ex-Mormons (who converted to Christianity but don't push it TOO hard in their writings) probably have the most historical evidence against the church. It is summarized in the following book http://www.utlm.org/onlinebooks/changecontents.htm but their website also has many sources http://www.utlm.org/navtopicalindex.htm.
