Name removal by email

Recently saw a post regarding name removal/resignation by email. I can't find it now. Did anyone save it, or is anyone familiar with how this can be done?

If you send a letter to SLC, is it to the address in the resignation section of this site, or is there a Greg Dodge separate address?
