Steve Benson's KKK in Utah post was great. I noticed that Salt Lake City banned "masks" as a way to deter the Klan. I wonder if this is the origin of discouraging mask on Holloween? Any ideas about this?

It actually is because of a bit of little known history / and anti-Mormon propaganda that was accepted as fact.
