Even polygamists who did not obtain church sanctioned permission for another marriage were disciplined.

I don't think Heber C Kimball was ever exed. Also, more current are missing, such as Richard Lyman (adultery or late polygamy, depending on view) and George Lee (various possibilities).

It's John W. Taylor, son of John Taylor the president. He was in the 12 and was ex'ed for refusal to give up polygamy.
