Would they have to tell their bishop they'll be masturbating into a cup? Would they be told to think about church things while they do it?

One of my relatives is an uber-TBM, and when he and his wife were going through the fertility clinic process, he would insist that she come in and help him fill his cup so that he wouldn't have to sully himself. She went along with it, but I think she thought he was being a little ridiculous. Incidentally, he was one of those who thought one-piece garments were somehow holier than the two-piercers.
