No, you don't need your membership number. Your name and the date/place you were baptised along with your birthdate is enough.

Congratulations! Don't be surprised when you get a letter back from Greg Dodge saying its a local matter and he has sent it to your local bishop who will contact you. Even if you requested no contact. It's their standard operating procedure and their own red tape. Once he gets it, your out!
